Does anyone have a good format/example of a counseling for Misuse of government travel card.

It has been brought to my attention that you may have violated AR 600-20 with regard to use of your Government Travel Card. Specifically that on or about _____________(date/time), you made the following purchase or purchases ___________(describe purchase), for $_______(insert dollar amount). As a result I will conduct/initiate a formal/informal (circle one) inquiry regarding this purchase or purchases. Should the outcome of this inquiry reveal that you did in fact violate the parameters of use for your travel card I will make a recommendation as to how the situation should be resolved. Abuse or misuse of the Government Travel Card is an action that is punitive in nature. Soldiers violating this policy may be subject to Article 92 of the UCMJ (Failure to obey a lawful general order or regulation), Article 133 (Conduct unbecoming an officer and a gentleman), and Article 134 (Debt, dishonorably failing to pay).
