If you want to save yourself some time and develop yourself professionally consider purchasing the following products: Counsel quick volume 3 and Counsel Quick Volume 1. These products specifically address: disrespect, failure to obey an order, and failure to report.

But to address your situation I have put together a quick example that may be of use to you. The issues to cover would be disrespect and disobeying an order or failure to follow instructions

For example: _____(Insert Rank/Name) on or about _____(insert date/time). You failed to follow instructions in that you showed up to the 0900 formation wearing ACU's when you were specifically directed to report in PT uniform. In addition (CHOOSE ONE) you were disrespectful to:_______________(insert name and rank) in that you _____(state what happened) or your attitude and tone conveyed a disrespectful mentality when I addressed your failure to be in the correct uniform. This was evident by the fact that you rolled your eyes and made unprofessional statements, specifically that you_____(Insert specifics).

As a result of the behavior noted above I am recommending the following actions (Pick one or more): UCMJ, revocation of privileges, or corrective training.

Finish with standard derogatory statement.

Corrective training could include:
1. Showing up every hour on the hour in a different uniform for a specific period of time.
2. writing a 500-1000 word essay on why it is important to follow instructions and why respect is important or how lack of respect degrades the ability of the Army achieve its mission
3. Give a 5 min class on why it's important to following instructions.

Please realize this is a rough draft and would need to be modified to fit the situation. I Hope this information has been helpful and please feel to contact us if you have any other questions. We are here to help. Best Wish
