On the NCOER support form do you use the same support form each time you counsel the soldier or start a new form?

You would use the same one and make additions/adjustments as necessary. We also recommend using a DA Form 4856 as well to document the counseling in detail. The NCOER support form would be a summary of the 4856 using bullets.

If you have the Rater Quick NCOER Edition you can whole process in the NCO Counseling Process chart in the Counseling Menu.

What I do is I keep the first page of the 2166-8-1, and for each QTR I do a diffrent page 2, so each counsleing session will have what they were expected to do, and then the Bullets that they actually achieved for that QTR at the end of the year the NCOER is complete, I just take the bullets I compiled through the QTRs and I am done.
